They are most often found on the scalp. The cysts are smooth, mobile and filled with keratin, a protein component found in hair, nails, skin, and horns. They are, however, clinically and histologically distinct from trichilemmal horns[clarification needed], which are much more rare and not limited to the scalp. Trichilemmal cysts may run in families and they may or may not be inflamed and tender, often depending on whether they have ruptured. Rarely, these cysts may grow more extensively and form rapidly multiplying trichilemmal tumors, also called proliferating trichilemmal cysts, which are benign but may grow aggressively at the cyst site. Very rarely, trichilemmal cysts can become cancerous Trichilemmal cysts are derived from the outer root sheath of the hair follicle. Their origin is unknown, but it has been suggested that they are produced by budding from the external root sheath as a genetically determined structural aberration. They arise preferentially in areas of high hair follicle concentrations, therefore, 90% of cases occur on the scalp. They are solitary in 30% of cases and multiple in 70% of cases. Histologically, they are lined by stratified squamous epithelium that lacks a granular cell layer and are filled with compact "wet" keratin. Areas consistent with proliferation can be found in some cysts. In rare cases, this leads to formation of a tumor, known as a proliferating trichilemmal cyst. The tumor is clinically benign, although it may display nuclear atypia, dyskeratotic cells, and mitotic figures. These features can be misleading, and a diagnosis of squamous cell carcinoma may be mistakenly rendered. Treatment Surgical excision is required to treat a trichilemmal cyst. The method of treatment varies depending on the physician's training. Most physicians perform the procedure under local anesthetic. Others prefer a more conservative approach. This involves the use of a small punch biopsy about 1/4 the diameter of the cyst. The punch biopsy is used to enter the cyst cavity. The content of the cyst is emptied, leaving an empty sac. As the pilar cyst wall is the thickest and most durable of the many varieties of cysts, it can be grabbed with forceps and pulled out of the small incision. This method is best performed on cysts larger than a pea which have formed a thick enough wall to be easily identified after the sac is emptied. Small cysts have walls that are thin, and easily fragmented on traction. This increases the likelihood of cyst recurrence. This method often results in only a small scar, and very little if any bleeding.

Join us on a deep Tibetan Street Food Tour! You can travel with us through this street food tour vlog and taste some of the most UNIQUE, delicious (LIKE YAK SASHIMI!), and UNKNOWN street food around the world. We traveled to Kangding, specifically to eat the best street food. First up, we went to a morning street food market and found a ton of different Yak, all laying out and ready to sell to chefs for their special Chinese/Tibetan recipes. This is the same YAK that we sampled for the YAK SASHIMI at a fancy Tibetan restaurant in the evening. We went to a Tibetan breakfast restaurant and had some amazingly powerful Yak organ soup, which to be honest, was not very delicious, although the spicy chili sauce was great! After that, we traveled to the local monastery buddhist temple to eat with the monks! This was truly a once in a lifetime street food temple experience! The authentic local Chinese Tibetan recipes the chefs were making were incredible! The flavours will blow you away, they all had this incredible home-cooked flavour. There were so many authentic Chinese recipes that the chefs were making, it will blow you away! All of it had an amazing home-cooked flavour! After that amazing monastery meal, we went back into downtown on the hunt for delicious street food recipes to try, and found an amazing Tibetan restaurant serving up some amazing Tibetan delicacies, like raw yak meat Sashimi, tsampa highland barley flour with yak butter and so much more! You can follow our travels through this tea horse road series as we travel through China on a 6 week street food tour journey and try delicious Chinese street food and Chinese cuisine all the way from Sichuan and into the west of Sichuan with Tibetan food, and then into Yunnan in the south of China!
